New Student Enrollment: Account Request

This form is the first step to enrolling your new student online. Complete it to request an account that you will use to log in to a secure Online Enrollment system.
Welcome! PLEASE NOTE: Enrollment for the 2023 – 2024 school year is open. Enrollment for the 2024-2025 school year will open April 1, 2024. Completed registration applications take approximately 5-7 business days to process. Applications are not considered complete unless all proof of residency documents are received. If proof of residency documents are not attached, your application will be further delayed. Once all required proof of residency documents are received, the registration application will be processed. You will receive a confirmation email after the registration has been finalized. You will then be contacted by your home school. Please complete the required fields to request an account to enroll your students and attach the required Proof of Residency documents. If you need further assistance, please contact our Central Registration Office by emailing registration@d428.org or call 815-754-2993 (please allow 2-3 days for a reply.)
